What Are the Basics of BOTOX ® (onabotulinumtoxinA)?

If muscle activity is the problem behind certain wrinkles, minimizing that muscle activity is the solution. BOTOX® Cosmetic, an injectable in the neuromodulator family, interferes with the proper transmission of nerve signals.

When the muscles rest, so does the overlying skin. After injection with BOTOX®, the targeted muscles stay more relaxed instead of receiving the message to contract fully. For a natural look, “less can be more” with judicious dosage and placement.

BOTOX® is used for …?

The FDA allows Allergan to manufacture and promote BOTOX® as a treatment for horizontal forehead lines, and glabellar frown lines (between the eyes). It also approved it for crow’s feet (“smile”, or “lateral canthal” lines) adjacent to the eyes.

Years of research and practical use by talented doctors have also led to other successful applications for BOTOX®. These include brow shaping, and raising a droopy brow,  bunny lines on the nose, visible gum-line when smiling, and rough/dimpling chin texture (“pebble-“, or peau d’orange chin).

Excessive, uncontrollable sweating, better known as axillary (under-arms), palmar (hand palms) and plantar (feet bottoms) hyperhidrosis is yet another application.

What is a BOTOX® Session Like?

After your complimentary consultation is through and you have decided on BOTOX®, the actual treatment will go quickly. Each visit has unique elements that depend on specific details, but generally involves only a few injections, completed in under 10 to 15 minutes. Expect the feel of a slight “pinch” with each injection, but little more in the way of discomfort.

There may be some soreness, redness, swelling, or an occasional tiny bruise at the injection site. These and other, similar side effects do tend to resolve on their own without intervention. A touch-up or ‘tweak’ may be done after a two-week interval, especially if this was your first treatment. But then we have an established pattern for you going forward!

How Long Does BOTOX® Last?

The effect of a BOTOX® treatment will begin to become visible within a few days of the injections and kick in fully at 10 to 12 days. From here you can realistically expect the wrinkle-reducing results to last for three to five months. Throughout that time, activity in the treated muscles will be inhibited. However, as the effects wear off, these muscles will eventually contract normally again. The motion will return, and the related folds and lines eventually come back.

Something to keep in mind: Botox® and two of the other Botulinum toxins can induce an immune response in your system. As a result over time, you would become somewhat resistant. We can try and overcome this by increasing your dose (treating more often would make the issue worse). This is known as dose-creep and occurs in up to 14% of patients.  We can also switch to Xeomin (incobotulinumtoxinA) which has never been reported to have this issue. The reason is explained in the link. Please bear in mind that it may take a few cycles to wash out your existing antibodies. As that process is allowed to take place, the Xeomin becomes more efficient and you may experience the response you initially got with your other botulinum product.

How Much Does BOTOX® Cost?

On an everyday basis, our Botox® price is $14 per unit.

There are ways to save on the cost of treatment. For patients who want to keep away their forehead lines, crow’s feet, and other wrinkles we offer long-term cost-savings. This is the Mind Your Skin Maintenance Program. With injection sessions scheduled at regular 3 1/2 – 4  month intervals, you maximize safe, reliable results to help you keep looking your best.

In addition, MindYourSkin and Dr. Robben has partnered with Allergan, the makers of Botox, and help you save through their rewards program named Allē. As of recent however, Allergan has made it very difficult to convert points to cash as used to be the case.

Go to www.Alle.com and  start an account, if you don’t already have one. That will also work for Juvéderm®, Kybella® and Latisse®, as well as for various in-office treatments. Late August, patients received an email from ALLĒ stating:

“This fall, we’re changing the way you use Allē points—giving them more value than ever before. You can use them on a handful of qualifying products and treatments from skincare to facial injectables, allowing you to get the treatments you love and those you’ve been wanting to try if appropriate.”

We were only made aware with three days to go that something was left out of Allē’s announcement. This is the fact that most of you will no longer get to exchange points for dollars as you have done in the past. Instead, points will accumulate but can only be used when they reach certain levels. And then only on specific Allergan skin care or services. This ‘enhanced program’ started Sunday September 30 at midnight!

Unfortunately, for most of our patients, Allē’s new program features will not result in any practical benefit.  Unless there is a significant increase in spending first. Instead then, chances are that the points/dollars will permanently flow back to AbbVie/Allergan/ALLĒ. Please note: This does not apply to gift certificates.

We are just passing on this information. We have no control over the program. After years of working with ALLĒ however, we are very disappointed with their decision and the process.

Besides  BOTOX® (onabotulinumtoxinA) in Philadelphia, meet the other members of the neuromodulator family: Dysport®, Jeuveau™, and Xeomin®. All have some unique properties setting them apart, but they function essentially the same wrinkle-smoothing way as BOTOX®. We are not convinced the treatment option Daxxify (daxibotulinumtoxinA-lanm) or ‘DAXY’ provides a better cost-benefit compared to the options (above) that we already offer.

BOTOX® works out  “dynamic wrinkles” because these are associated with skin movement caused by repetitive muscle activity. “Static wrinkles” develop under the influence of gravity, due to a drop in collagen, elastin and hyaluronic acid levels. These wrinkles and folds require different cosmetic treatment to address.
